- Register the product on Orbit’s website at www.orbitbaby.com/register by 6/30/2011
- Send an email to promo@orbitbaby.com containing the following:
- Name of Equinox Fitness Club location where you attended the Mom Appreciation Day event
- Scanned image or photo of your purchase receipt (showing product purchased, retailer name, and date of purchase between 5/1 and 5/15/2011)
- Your preferred color of Bassinet Cradle G2 (Black, Mocha, or Ruby)
- Your shipping address
